Referral Process
Welcome to CHS!  Finding CLS services for your clients has never been easier.  CHS staffing referrals are now being accepted through a new collaborative process between ExpertCare and CHS Group.  ExpertCare will be receiving and managing all CMH referrals that require staffing support for CHS Group. This change is intended to streamline the referral process and improve efficiency and communication for all parties involved.

​We have implemented a new fillable referral form that will replace the previous CHS referral process. Once the referral form is completed, it can be submitted directly to ExpertCare’s Referral Department. Any supporting documentation—such as IPOS, behavior plans, or related materials—can be uploaded and submitted along with the form.
What to expect next: 
  • Referrals are submitted using the new fillable form https://tinyurl.com/bdr432kv (or just click the button at the top of the page!)
  • Supporting documents can be attached at the time of submission
  • ExpertCare will acknowledge receipt and follow up within 24–72 hours
We recognize this is a change from the previous workflow and appreciate your flexibility as we transition. Our goal is to make this process as simple, clear, and efficient as possible while continuing to support your staffing needs.
